What's the difference between Letter of Approval from MDEC or Immigration.
If your employer in Malaysia is MSC (Multimedia Super Corridor) status then they can applied your visa through MDEC. MDEC is an agency established by Malaysian government overseeing MSC implementation. One of it's function is to cater Employment Pass/ Visa application. It reviews the application and decides if it's endorse to Immigration or rejects. It streamline the process which make the turn around faster.
Your company/ agency will apply through MDEC, endorse to Immigration once approve. Employment Pass/ Visa approval will come into digital format or soft copy. Normally the company will email the pdf format of approval letter with MDEC logo

If the company applied directly to Immigration, this may take a while since it will go through a long process of review and approvals. Once it get approve, the approval letter sealed in envelope is usually couriered intended for Malaysian embassy. In which the candidate must bring together with his copy to Malaysian embassy when filing for single entry visa.
